Telehealth consent (draft)

Draft · pending counsel and operator review

This consent is not active. Clinical services have not launched and no one can agree to treatment through GetPeptides today.

When services launch, before any visit you would be asked to agree, separately and in plain language, that: care is delivered remotely by an independently licensed clinician; remote care has limits and may not be right for everyone; the clinician may decline to treat or prescribe; you will provide accurate information; electronic communication carries privacy risks; you can withdraw consent at any time; and in an emergency you will call 911.

Before activation, the treating entity must identify the licensed clinician, confirm patient location and applicable state authorization, explain visit modality and technology limits, offer alternatives and withdrawal procedures, set emergency/escalation steps, disclose information sharing and costs, and record a versioned consent. GetPeptides will not decide whether a prescription is appropriate. The treating clinicians' own consent terms will govern actual care. [Review: medical-group consent, state-specific requirements.]

Draft pending counsel and operator review. Business details are owner-supplied, not independently verified. Items marked [Review] require verification of registration, address spelling, contact processes, governing law, partners and state scope before launch. Nothing here claims certification, licensure or regulatory approval.
